Position Summary:
The Director of Health & Wellness is responsible for overseeing the overall clinical operations and wellness of residents within the Assisted Living and Memory Care Community. This leadership role ensures quality resident care, regulatory compliance, team development, and collaboration across departments to support an exceptional resident experience.

- Eligible to earn 10% of annual salary in a yearly bonus
- Sign-on bonus of $10K paid out at the rate or $2500 every 90 days for the first 12 months.
Essential Duties and Responsibilities:
The following duties are representative of the position and are not intended to be all-inclusive. Additional responsibilities may be assigned as needed.
- Oversees and ensures compliance with all federal, state, and local regulations related to resident clinical care and wellness services.
- Provides leadership and direction to licensed nurses, caregivers, and clinical support staff to ensure delivery of high-quality resident care.
- Conducts resident assessments and oversees the development, implementation, and ongoing updates of resident service plans.
- Monitors residents for changes in condition and ensures appropriate communication and documentation regarding care needs.
- Collaborates with interdisciplinary team members to evaluate and adjust care plans, medication management, and service levels as needed.
- Maintains effective communication with residents, families, physicians, pharmacies, and responsible parties regarding care updates and health concerns.
- Partners with the pharmacy consultant to ensure optimal pharmaceutical services and medication management practices.
- Oversees incident reporting, analyzes trends, and implements corrective action plans and quality improvement initiatives when necessary.
- Partners with the Executive Director and leadership team to support resident safety, infection control, and overall clinical excellence.
- Assists with staff education, clinical training, and ongoing competency development.
- Oversees monthly wellness documentation and ensures timely completion of required reporting.
- Manages clinical staffing schedules and staffing coverage to ensure resident care needs are met.
- Maintains appropriate inventory and availability of clinical supplies and equipment.
- Participates in community outreach, resident/family engagement, and community awareness activities.
- Participates in a rotating on-call schedule to support clinical operations and resident care needs.
- Performs other duties as assigned.
Supervisory Responsibilities:
Provides direct supervision and leadership to LPNs, Certified Nursing Assistants, Medication Aides, and other clinical staff as assigned. Responsible for coaching, performance management, training, scheduling, and ensuring consistent delivery of quality resident care in accordance with company standards and applicable regulations.

Educational Requirements and Experience:
- Registered Nurse or Licensed Practical Nurse in good standing; Registered Nurse preferred
- One (1) year in a nursing leadership role; Senior Living experience preferred.
- BSL Certification

- Must demonstrate an interest in working with a senior population.
- Ability to communicate effectively speaking the primary language of the residents.
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
- Ability to write routine reports and correspondence.
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
- Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
- Interacts with guests, residents and staff in a courteous and friendly manner.
- Responds promptly to resident needs.
Environmental Adaptability:
- Works primarily indoors in a climate controlled setting.
- Team Members may be exposed to blood and/or body fluids with potential exposure to hazardous materials and infectious diseases.
- Possible exposure to unpleasant odors.
- Possible exposure to chemicals as identified in the MSDS Manual.

The employee must regularly lift and /or move up to 50 pounds independently and up to 100 pounds with assistance of a second employee and/or mechanical devices. Must be able to push up to 350 lb. (wheelchair). Specific vision abilities required by this job include close vision and distance vision.
