Position Description: Home Care Aide
Status: Part-Time With Benefits: 24 - 29 Hours per Week. Potential of moving to full-time.
Shift: Day - 8:00 a.m. to 4:30 p.m.
Days: Monday thru Friday; including 1 weekend per month

Education/Qualifications: High School Diploma or equivalent; Must have 75 Hour CNA training and certification. Requires current driver's license and auto insurance, with the ability to travel throughout Boone County.
Essential Functions:
- Regular attendance.
- Demonstrates knowledge and skills necessary to provide care appropriate to the ages of the patients served within the department.
- Maintains performance in compliance with State and Federal laws, and consistent with high standards of business and professional ethics.
- Provides personal care assistance to patients such as bathing, grooming, dressing, simple wound cares, ambulating, and rehabilitation.
- Assists patients with oral medications as directed by home care professional.
- Provides meal planning and preparation as directed by supervisory staff.
- Provides basic light housekeeping such as sweeping, mopping, vacuuming, dusting, cleaning bathroom/kitchen areas, changing linens, and laundry.
- Provides miscellaneous duties such as grocery shopping, errands, money management/bill paying, and parenting skills.
- Documents services provided according to documentation standards of the department.
- Reports changes in patient condition to supervisor.
- Maintains effective communication with all levels of supervision and staff.
- Maintains HCA scheduling planner, and attends regular scheduling meetings.
- Attends and participates in department staff meetings/in-services and hospital wide mandatory in-services.
