As a full time, nonexempt CPS Solutions LLC MTM Pharmacist at Munson Healthcare Specialty Pharmacy, you will work with the specialty pharmacy team and within the assigned clinic(s) of the health system. The Pharmacist is responsible for performing comprehensive and targeted Medication Therapy Management (MTM) in high-risk patients to identify, resolve, and prevent medication-related problems. The Pharmacist documents the care delivered and communicates essential information to the patient's primary care providers and the health care team. The Pharmacist is also responsible to follow processes to improve patient care, outcomes, and quality improvement metrics. This position will report to the Specialty Pharmacy Manager.
Schedule: This position is scheduled to work at least 40 hours per week, Monday through Friday day shift. There are no nights, weekends or holidays.
Starting Pay: $65 - $67 per hour, (Depending on related work experience).
About the Facility: The Foster Family Community Health Center, formerly called the Munson Community Health Center, is the front door to services provided at Munson Medical Center and elsewhere in the Munson Healthcare system. People from around northern Michigan use Foster Family Community Health Center for pre-operative testing, pain management, rehabilitation therapies, urgent care, sleep disorders, and many other diagnostic services. CPS Solutions, LLC has partnered with the hospital to manage the day-to-day operations of this pharmacy. This position is an employee of CPS Solutions, LLC.
About the Pharmacy: The Specialty Pharmacy will utilize PioneerRx or a similar program. The Pharmacy hours will be Monday-Friday from 9am to 5pm.
Required Qualifications:
Pharmacy degree (BS or PharmD) from an ACPE accredited school of pharmacy; PharmD preferred
Current Pharmacist license in good standing in Michigan
Controlled Substance licensure in the state of Michigan
Successful completion of a Pharmacy Practice Residency program strongly preferred
Previous experience in specialty pharmacy and/or outpatient pharmacy preferred.
Exposure to medication therapy management preferred.
Proficient computer skills using Pharmacy systems and Microsoft applications – Outlook, Word, Excel.
Effective communication skills - verbal and written
Through training and/or experience, possess necessary knowledge to provide oversight of medication regimens for the patient populations treated in the facility including knowledge of dosing for specific age populations.
Specialized Knowledge/Skills:
Ability to use pharmaceutical and medical references literature to provide drug information to health plans, patients, and prescribers.
Excellent written and verbal communication skills
Excellent relationship building skills.
Strong interpersonal skills.
Ability to work independently, as well as a team.
Hospital Requirements: (may be required)
- (PPD) TB Skin Test - Proof of negative TB skin test within the last 12 months
- (MMR) Measles, Mumps and Rubella or A Blood Titer proving immunity
- Varicella - (2) documented doses or A Blood Titer proving
- Hep B3 Series (or declination)
- (Flu) Influenza-required for hire between Oct 1st-April 30th
- COVID Vaccine
