**Make a Difference**
The Hospice LPN works closely with RN Case managers to meet the needs of hospice patients and their families in their home and in long-term care facilities. The Hospice LPN demonstrates performance consistent with professional standards of practice, care, performance, the Nurse Practice Act and the mission and goals of the organization. Manages patient care in the health care continuum to achieve optimum outcomes in a safe and cost-effective manner. The Hospice LPN will provide patient care in the North region: Indianapolis - Anderson, IN.
**Exceptional Skills and Qualifications**
· Compassionate as a professional caregiver.
· Excellent organizational skills.
· Strong critical thinking and communication skills.
· Demonstrates ability to respectfully address interpersonal conflicts.
· Demonstrates customer service skills to provide exceptional patient, and family experience.
· Graduate of a recognized accredited school of practical nursing, or Graduate from a non-accredited school of practical nursing with three years related professional nursing experience.
· Licensed as a Licensed Practical Nurse (LPN) by the Indiana Professional Licensing Agency required.
· Current Indiana Driver's License required.
