Job Title: RN Case Manager Assistant
Job Description
The RN Case Manager Assistant provides comprehensive case management for patients at skilled nursing facilities. Responsibilities include coordination with SNF team members, nursing, rehab, discharge planning, dietary, admission, DON, administrator, and KP physicians. You will conduct medical necessity screening and collaborate with the interdisciplinary team to provide care coordination under the direction of a Registered Nurse, adhering to evidence-based practice and regulatory requirements. This role integrates national standards for case management, focusing on Utilization Management, Transition Management, and Care Coordination to ensure patient satisfaction and compliance with state and federal regulations.
Hard Skills
RN license
Discharge planning experience
Acute hospital setting experience (200-300 bed hospital)
Clinical experience in a hospital
HealthConnect or EPIC experience
Excel data entry
Soft Skills
Clear, complete, and concise documentation
Effective communication with interdisciplinary team
Collaboration with physicians, office staff, and ancillary departments
Advocacy for patients and hospital
Balancing cost and quality for optimal outcomes
Job Type
This is a contract position with a duration of 6 Month(s).
Work Site
This is a fully on-site position in Ontario, California.
Work Environment
M-F with rotating weekends (every other weekend), 8am-5pm
