Position Overview:
We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide nursing care to patients in accordance with the nursing plan of care and will be committed to advocating for the health and well-being of our patients.
Key Responsibilities:
Provide direct patient care, including monitoring vital signs, administering medications, and assisting with daily living activities.
Assist in the development and revision of nursing care plans based on patient needs and progress.
Maintain accurate and timely documentation of patient care activities and outcomes.
Communicate effectively with patients, families, and healthcare team members to ensure optimal patient care.
Assist with patient education and provide information on treatment plans, medication, and health management.
Adhere to all safety, infection control, and health regulations/policies.
Qualifications:
Valid state licensure as a Licensed Practical Nurse (LPN).
Previous experience in a clinical setting preferred (e. g., skilled nursing facility, hospital, or home health).
Basic Life Support (BLS) certification is required.
Strong communication and interpersonal skills.
Ability to work effectively in a team-oriented environment.
Compassionate, patient-focused, and efficient in delivering care.
