Overview
Licensed Practical Nurse (LPN) - Private Duty - Full-Time
Location:Chattanooga, TN
Job Overview:Are you a compassionate and dedicated Licensed Practical Nurse (LPN) seeking a rewarding role in private duty nursing? Adoration Health is looking for a full-time LPN to join our team in Chattanooga, TN. Provide individualized care to a geriatric patient in the comfort of their own home, ensuring the highest quality of care.
Key Responsibilities:
Deliver high-quality nursing care, including vent/trach management and catheter care.
Administer medications, treatments, and medical procedures as prescribed.
Monitor and document patient's condition and progress.
Communicate effectively with physicians, patient, and family members.
Educate the patient and their family on health care and self-management.
Maintain accurate patient records and ensure compliance with HIPAA regulations.
Schedule:
Hours: Monday - Friday, 9 AM - 5 PM
Qualifications:
Active LPN license in the state of Tennessee.
Minimum of 1-year nursing experience, preferably in home health or private duty.
Extensive experience with vent/trach and catheter management.
Strong clinical and assessment skills.
Excellent communication and interpersonal skills.
CPR certification.
Ability to work independently and manage time effectively.

Responsibilities
- Perform selected nursing procedures within the scope of LPN practice- Provide patient and family education according to plan as outlined by the RN- Monitor patient's vital signs and make specific measurements and observations- Monitor and record intake and output, and report significant changes immediately- Observe, assess and record patient's physical and mental condition and report any changes- Administer medications as instructed and ordered by the physician and instructed by the RN- Attend and participate in staff meetings and in-service education programs, as assigned
Qualifications
- Active LPN license in good standing- Previous home health, private duty, skilled nursing or hospital experience preferred- Ability to build rapport with patients and family members- Strong problem solving and critical thinking skills- Reliable transportation or valid driver's license with current automobile insurance
