Compassionate In-Home Private Duty LPN - Full-Time, Nights
Location:Newbern, TN (Patient's Home)
Schedule:3-4 Split-36 Hours Alternating with 48 Hours/Week, Every Other Weekend, 7:00 PM to 7:00 AM
Job Description:
Join our team at Adoration Health as a Licensed Practical Nurse (LPN) for a fulfilling full-time private duty nursing role in the Newbern, TN area. This position involves providing specialized care, including trach and g-tube management, to a patient in their home environment during night shifts.

Key Responsibilities:
Deliver compassionate, personalized care to the patient.
Manage trach and g-tube care as required.
Administer medications and treatments as prescribed.
Monitor patient's health and update medical records.
Collaborate with the healthcare team to provide comprehensive care.
Requirements:
Current LPN license
Experience with trach and g-tube care is required
Strong communication and interpersonal skills
Ability to work independently and manage time effectively

Responsibilities
Perform selected nursing procedures within the scope of LPN practice
Provide patient and family teaching according to plan outlined by the RN
Monitor patient's vital signs and make specific measurements and observations
Monitor and record intake and output. Report significant changes immediately
Observe, assess and record patient's physical and mental condition and reports any changes
Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned
Qualifications
Licensed Practical Nurse / LPN license in good standing
Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
Active CPR certification
Familiarity with electronic health records
Strong problem solving and critical thinking skills
Reliable transportation, valid driver's license and current automobile insurance
