Summary The Patient Aligned Care Team (PACT) Registered Nurse (RN) Care Manager will utilize the nursing process and evidence based practice to deliver and manage comprehensive care to an assigned panel of patients in a clinic setting. The RN is a key member of the PACT and multidisciplinary team and is responsible for collaboration with other team members, promoting effective communication and ethical practice. VA offers a comprehensive total rewards package: VA Nurse Total Rewards The Patient Aligned Care Team (PACT) Registered Nurse (RN) Care Manager demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. The nurse collaborates with the Core Team (Patient, Primary Care provider, PACT/RN Care Manager, PACT LPN and Medical Support Assistant) and Expanded Team (family/caregiver, internal and community based services in providing care to the patient) in developing the patient driven holistic care plan for life. The nurse collaborates with services internal and external to the VA to facilitate care transition to effectively meet the patient's needs. The PACT RN also functions as a part of the broader Primary Care Clinic team and has effective customer service skills when working with internal and external customers. The primary duties for the Patient Aligned Care Team (PACT) Registered Nurse (RN) Care Manager position include, but are not limited to, the following: Provide prescribed medical treatment and personal care services to ill and injured Veterans . Provides efficient care triaging patients documenting observations, assessments, and changes in patient's condition. Collaborating with health team members to facilitate positive patient care outcomes. Providing a full range of nursing care to patients with a variety of physical and/or behavior problems. Educating patients about healthy practices and treatment modalities. Coordinating the care across settings and among caregivers and managing information. Communicating effectively with internal and external customers Complies with health, safety and environmental rules and procedures. Performs work in a manner that enhances the safety of the work environment. Ensures strict adherence to federal, state, and local codes concerning health, safety and the environment as applicable within the performance of his/her duties. Assist each patient to identify and meet basic health needs and improve his/her health status to the individual's maximum potential. Participates in unit/program level quality improvement processes and initiatives, as well as, customer service program. Qualifications Basic Requirements: English Language Proficiency. In accordance with 38 U. S. C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6); OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD).
