This is an outpatient nursing role with the Division of Pediatric Endocrinology which is a joint division between Cooperman Barnabas Medical Center and the Children s Hospital of New Jersey (CHoNJ) at Newark Beth Israel Medical Center. The successful candidate will become a member of the diabetes education team that meets with pediatric diabetes patients and their families for teaching and diabetes management. In addition, this role supports the clinical endocrinology functions of the division at CHoNJ

Qualifications:

Required:

ASN or Nursing Diploma

Strong communication and organizational skills

Proficient computer skills

Minimum 1 year of acute or ambulatory outpatient experience

Preferred:

BSN

Pediatric experience

CDCES preferred but will train the right candidate

Certifications and Licenses Required:

Active NJ RN license or compact RN license with NJ endorsement

BLS from the American Heart Association

Scheduling Requirements:

Day Shift 37.5 hours 8:30a-4:30p, 9a-5p

Full Time

Monday Friday no weekends, no holidays

Essential Functions:

Assess the patient s past history and current condition, establish and monitor the patient/family goals and outcomes

Assess and implement patient/family education and home care needs

Use an understanding of the principles of growth and development to assess patients age specific needs and age specific treatment.

Specifically responsible for review and management of patient blood sugar levels, including monitoring patients who use Continuous Glucose Monitoring Systems and insulin pumps.

Informing patients of lab results, patient education tests such as training patients to administer medications at home, and administration of injectable medications in office as required.

Assisting with the attainment of prior authorizations as needed.

Arranging for infusion and specialized testing as needed.

Giving in-office injections as needed.

Teaching patients about how to administer injectable medications at home such as growth hormone

Diabetes Education Responsibilities

Newly diagnosed patients with diabetes Inpatient and Outpatient Education.

Topics Include:

How to use a glucometer, proper testing technique, when to monitor

Treatment of hypoglycemia

Treatment of hyperglycemia, ketonuria testing

Carbohydrate counting

Insulin action time and timing of injections

How to use Glucagon

Diabetes supply management

Diabetes in the school environment

How to calculate an insulin dose

Exercise and Activity

Newly diagnosed phone education and support daily at first, moves eventually to weekly

Education of additional caregivers when necessary

Additional education as needed including insulin pump topics, sensor education and insertion training

Assists with transition to college, provides supportive documentation regarding accommodations

School Care Plans beginning of each year every patient with diabetes receives a customized school care plan that goes to school. These plans have to updated throughout the year when insulin therapy changes. Support to the family as they go through 504 Plan process.

Communication with School Nurses regarding care plans and school related issues

Insulin Dose changes Between Physician Visits Review meter, pump and sensor data to determine insulin dose changes, consult with physician, communicate changes to parent/patient, assist with changes, update school paperwork, document changes in EMR

Diabetes Physician visits diabetes educator downloads diabetes device or prints reports, interviews, reports information to physician, interprets pump and sensor data, recommends insulin dose changes, provides additional diabetes education as needed, writes visit note

Sick Call (When patients are ill, have ketones, vomiting) diabetes educator speaks to the family/school nurses multiple times throughout the day assessing BG and ketone levels, consulting with physician, referring to ER if needed

Certificate of Medical Necessity paperwork for diabetes supplies, insulin pumps, and continuous glucose monitors
