We are seeking a Spanish speaking Diabetes Educator to support Lutheran Hospital.

We are in need of a Spanish-speaking diabetes educator at Lutheran hospital to support our current Endocrinology office. The goal of the Diabetes Center is to encourage patients to receive early specialty care and education - getting them on the right track with their diabetes management before returning them to their primary care physicians for ongoing management. In this role, you will be responsible for:

Coordinating the diabetes education program.

Providing educational resources.

Promoting and implementing services that achieve improvement.

The ideal future caregiver is someone who:

Is a Registered Nurse (RN) or Registered Dietitian (RD).

Is skilled in diabetes technology, such as insulin pumps and CGM.

Has a strong understanding of oral and injectable medications for diabetes.

Is comfortable conducting in person visits, virtual visits and group education.

Is skilled in addressing patient barriers and providing empathetic support for behavior change.

**Responsibilities:**

Provides direct patient care using medical nutrition therapy or diabetes self-management education for assigned patient populations requiring nutrition and/or diabetes support in which medications and monitoring are an integral component in the design of therapy.

Collaborates with Endocrinology providers to design and administer nutrition and diabetes care and education that supports patient safety in the setting of high-risk medication use.

Completes comprehensive assessments for each patient including emotional and behavioral health, knowledge level, disease history, desired outcomes, and electronic medical record data.

Develops an individualized care plan with safe and effective interventions based on the patient' assessed needs and goals, and promotes successful self-management through education and support.

Utilizes the most recent national standards and guidelines for diabetes and obesity care in practice, including the ADCES7 Self-Care BehaviorsTM.

Uses diabetes and weight management technology to assess diabetes status, provide patient care, and track outcomes.

Delivers specialized interventions and comprehensive counseling and education services to patients with diabetes and obesity who require nutritional management and diabetes care and education.

Implements a care plan, communicates plan, establishes a follow-up system (outpatient), and maintains documentation.

Provides complete, concise, and timely documentation of each patient encounter in the electronic medical record.

Develops and contributes to Cleveland Clinic sponsored education programs for physicians, nurses, allied health professionals, students, peers and the public.

Uses evidence-based guidelines to creates and revise education materials related to diabetes and weight management.

Participates in program development designed to market and expand departmental services and revenue generation.

Markets services to potential and current referral sources; develops and implements services in response to needs and wants of patients, referring providers, and administration.

Actively contributes to departmental initiatives, including committees, projects, and community outreach.

Completes reports for ongoing productivity and outcomes tracking.

Participates in professional growth activities; attends and contributes to conferences and seminars on diabetes, nutrition, and related topics.

Represents Cleveland Clinic on diabetes and nutrition related matters.

Supports departmental research and quality initiatives, and applies current research and evidence-based care to practice.

Adheres to department policies, procedures, quality standards and safety standards, and complies with governmental and accreditation regulations.

Other duties as assigned.

**Education:**

Bachelor's degree or higher in Nursing, Dietetics, or a closely related field.

Dietitians must have completed a dietetic internship through a credentialed program.

**Certifications:**

Maintains current license to practice dietetics or nursing in the State of Ohio

Dietitians must also have active certification as a Registered Dietitian/Nutritionist (RD/RDN).

Must earn and maintain certification as a Certified Diabetes Care and Education Specialist (CDCES) within two years of hire/assignment.

**Complexity of Work:**

Requires critical thinking skills, decisive judgment, and the ability to work unsupervised or with minimal supervision.

Must be able to work in a stressful environment and take appropriate action.

**Work Experience:**

Minimum one year experience as a Registered Dietitian or a Registered Nurse.

Connection/experience with diabetes program preferred.

**Physical Requirements:**

Ability to navigate extensively to provide services to a large number of patients throughout the hospital and clinic sites. Must have manual dexterity to sufficiently perform electronic data entry, and demonstrate devices and medications to patients.

Must have physical ability to measure and weigh patients.

**Personal Protective Equipment:**

Follows standard precautions using personal protective equipment as required.
