**Comprehensive Care**
Dominican offers emergency services and is a Certified Stroke Center and Chest Pain Center. Dominican's services include the only comprehensive Cancer Center in Santa Cruz County, a Total Joint Replacement program, and advanced neurological and endoscopic services. Dominican regularly receives 'A' grades for hospital safety from Leapfrog Group, and has received national recognition for superior patient safety, cardiac care, and stroke treatment from Healthgrades, a leading provider of comprehensive information about physicians and hospitals.
As Santa Cruz County's heart attack (STEMI) receiving center, Dominican Hospital provides cutting edge cardiac care. The hospital's heart attack treatment times consistently beat the national average, and the hospital offers two cardiac catheterization labs available 24 hours a day, 7 days a week. Groundbreaking cardiac procedures at Dominican include the Transcatheter Aortic Valve Replacement (TAVR) procedure, which allows for heart valve replacement without opening a patient's chest.
Dominican Hospital has forged significant partnerships with area hospitals to bring exceptional care to Santa Cruz County. The hospital offers a level III Neonatal Intensive Care Unit (NICU) through a partnership with Lucile Packard Children's Hospital Stanford. Neurosurgery at Dominican is also offered through a partnership with Stanford Health Care.
**In The Community**
Beyond care inside the hospital, Dominican is dedicated to the health and wellness of the Santa Cruz community.
Community programs include a Mobile Wellness Clinic, a tattoo removal program, and support groups for patients and new mothers. In addition, and in accordance with our mission, Dominican Hospital supports programs for the underserved in our community, including children and adults, students, seniors, and the homeless population. **Responsibilities**
The RN Care Coordinator is responsible for assessing planning facilitating and advocating for options and services through a continuum of care from admission to discharge for assigned patients. The RN Care Coordinator performs the above in such a manner as to meet the individual's health needs while promoting quality and cost-effective outcomes. The position's emphasis will be on care coordination communication and collaboration with nursing physicians departments within the medical center insurers and post-acute service providers to pace the care toward optimal outcomes within the appropriate level of care. The RN Care Coordinator will advocate for the patient and family by identifying and valuing patient choice spiritual needs cultural language and socioeconomic barriers to care transistions. In addtion the RN Care Coordinator will will protect confidentiality while striving to achieve high levels of patient satisfaction.
The RN Care Coordinator consistently conducts the utilization review process and discharge planning process per hospital policies standards of practice and by Federal and State Regulations. Experience: Required Minimum of Three years nursing experience in an acute hospital setting Prior Care Coordination experience in aclinical or insurance setting. If operational conditions permit training a candidate without the required experience may be considered. Preferred One year case management in an acute care hospital Education: Required Bachelors degree (current incumbents are exempt from this criteria. Preferred Masters degree in nursing with a concentration in Case Management may serve as a substitute for the experience requirement.
Training: Required Working knowledge of the RN scope of practice current state requirements CMS Conditions of Participation EMTALA Patient Bill of Rights AB 1203 and other federal or state regulatory requirements as specific to Utilization Review and Discharge Planning Fluent in English verbal and written skills Working knowledge of Microsoft Office and work related software Preferred Certification appropriate for the position (CCM ABQAURP or ACM) Bilingual skills Licensure: Required Current and active CA RN license Current BLS certification
**Qualifications**
**Requirements:**
Minimum of 3 years nursing experience in an acute hospital setting.
Excellent customer service and presentation skills are a must Strong interpersonal and written communication skills are essential Demonstrated ability to apply analytical and problem solving skills Demonstrated ability to manage multiple tasks or projects effectively Ability to work independently as needed with a high degree of detail orientation.
Ability to work efficiently in a fast-paced environment with changing priorities Current and active CA RN license
**Education:**
Masters degree in nursing with a concentration in Case Management may serve as a substitute for the experience requirement Prior Care Coordination experience in a clinical or insurance setting.
**Preferred:** CM certification

$77.53 - $95.55 /hour
