*The Impact You Can Make* ** The Registered Nurse is accountable for management and delivery of patient care to assigned patients in collaboration with the interdisciplinary team, and within the scope of practice delineated in the New York State Nurse Practice Act. Provides direct patient care, delegates/assigns responsibility as appropriate to others, and serves as leader of the interdisciplinary team in collaboration with and under the direction of the attending physician. (i. e., standing orders influenza and pneumococcal vaccine, telephone triage protocols, RN anticoagulation management, etc.) * Defines expected outcomes from the diagnosis, involving the patient, family, provider, and healthcare team in formulating measurable expected outcomes within a defined timeframe * Identifies and develops an ambulatory care plan for patients seeking care for health promotion, health maintenance, or health related situational problems (i. e., conducts daily clinical huddle with care team to review gaps in care and plan for discussion and implementation with patient and family/caregiver). * Facilitates care coordination across health care settings to maintain continuity of care to ensure patients and populations are directed to the appropriate level of care. * Collaborates with physician and advanced practitioner providers to provide patient centric care using Glens Falls Hospital's Core Values as a guide with all interactions. *Education/Accredited Programs* * Bachelor's degree in Nursing is preferred; new hires will be required to complete their BSN within 5 years of hire.**Exceptions may be made for Associate's Degree RNs with greater than 20 years of experience and national certification in specialty areas. /Does not pertain to existing GFH RNs./ * Graduation from a Registered Professional Nursing Program. * One year nursing experience required. * Maintains competency in professional nursing practices. * Assumes responsibility for own self-development and for keeping current with advances in nursing knowledge and techniques. * Assumes responsibility for maintaining current knowledge related to the age and development of the patient population in areas in which they work. ** *Licenses/Certifications/Registrations* * *BLS*certification required before completion of orientation in all areas of practice & maintained throughout employment * *ACLS*certification required before completion of orientation in Urgent Care and maintained throughout employment * *PALS*certification required before completion of orientation in urgent care and maintained throughout employment * License to practice nursing in NY State without restrictions ** *Skills/Abilities:* * Ability to assume leadership role in nursing practice. * Ability to organize work, to delegate effectively, and to direct and oversee the work of practical nurses, nursing aides, and unit secretaries. * Ability to communicate effectively, verbally, and in writing with other nursing personnel, patients, and physicians. * Ability to complete basic mathematical calculations. ** *Communities We Serve* Located in the foothills of the beautiful Adirondack mountains, Glens Falls is conveniently located a short drive away from the capital region and Lake George.
