Description
Summary
Provides front line oversight to participants/members in the Baptist Well-being Program assisting in the development of strategies to improve the health status of defined populations. Provides guidance and direction to program participants in achievement of specific chronic disease management objectives to improve health outcomes and status. Utilizes a number of population health management tools and analytics to provide education and resources so that program members are actively involved in the management of their own health and well-being. Partners with health care providers to monitor care coordination efforts for both incremental and long term improvement(s).
Responsibilities
Serves as direct contact with members and within the designated service area.
Develops and implements educational offerings working in partnership with care teams to identify specific needs to support and promote the advancement of chronic disease management.
Completes assigned goals.
Requirements, Preferences and Experience
Education
Minimum: Bachelor's degree in health related field (Psychology, Exercise Science, Nutrition, Wellness, Health Education) or related experience. Knowledge of disease management tools and related data.
Experience
Minimum: 2-3 years of experience with overall work-site wellness and implementing health and wellbeing programs.
Special Skills
Minimum: Excellent interpersonal skills and ability to develop and foster collaborative partnerships with a variety of key stakeholders. Strong organizational and time management skills. Ability to prioritize effectively in a high volume environment. Must be available to work variable shifts such as evenings and weekends as needed.
Training
Preferred: Certified Health Education Specialist (CHES), Certified Personal Trainer (CPT) or other related health certification preferred.
Minimum: CPR and AED training
