Salary Rate: $55,000


Duties & Responsibilities 

  • The Lead Reentry Facility Case Manager supports the Facility Case Managers in the areas of Outreach/Recruitment, Assessment/Eligibility, work readiness skills, and the development of basic remedial skills in math and reading for the program and participants. 
  • Must be willing and able to work inside a correctional facility providing one-on-one, group instruction, case management services, and support Facility Case Managers.
  • Lead Reentry Facility Case Manager  will support and assist in meeting personal and team goals related to the program and training new and current staff on best practices for recruiting, managing caseloads, and completing follow-up. 
  • The Lead Reentry Facility Case Manager will recruit and enroll a specialized group of program participants, facilitate groups, and conduct one-on-one case management. 
  • Collaborate with Area Manager to assess and plan training needs and requirements; serve as point of contact for facility staff.
  • Confirm eligibility and manage enrollment activities, including scheduling and facilitating information and intake sessions in coordination with managers; maintain participant case records and assist others with case management.
  • Deliver skills training in software applications (in the community), work readiness skills, and the development of basic remedial skills in math and reading. Develop presentation and facilitation skills to engage students in meeting program goals.
  • Deliver program curriculum/objectives in classroom/simulated workplace environment; assist with NCWorks registration and other pre-release referrals.
  • Support Employer Engagement Specialists to coordinate services that address employer needs of the special populations.
  • Monitor client correctional facility release dates and coordinates reentry transition planning and participant transfers. 
  • Collaborate with Area Manager to plan, implement, and coordinate events to increase the success of participants in obtaining and retaining employment (such as mock interviews, job fairs, etc). 

Qualifications

  • Bachelor's Degree in Criminal Justice, Social Work, or related field (preferred), or an Associate's Degree in combination with experience including lived experience.
  • 4+ years of full-time experience working with the justice-involved, peer support, or project management preferred.  
  • Supervision, communication and organizational skills. Work independently and make decisions within the framework of established guidelines, as well as work in a team environment.  Ability to work without immediate supervision in direct one-on-one and small group instruction.  
  • Have an understanding of special populations. Accept, understand, and relate to clients who have behavioral, emotional, learning, or physical disabilities.  
  • Required to pass a drug screen in compliance with our Substance Abuse and Drug-Free Workplace policy.
  • Must be able to meet requirements for Eckerd Auto Insurance and drive for business purposes.
