Valley Health Systems, the parent company of Holyoke Medical Center (HMC) and Holyoke Medical Group (HMG) provides a broad range of primary care services in offices conveniently located throughout Western Massachusetts. Services include adult primary care, family medicine, pediatrics, and walk-in care. Our Adult primary care practice at 10 Hospital Dr, Holyoke, MA, is seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide nursing and care coordination for patients.# Responsibilities: Triage patients through direct contact, or messaging. Determines the clinical priority by assessing patient conditions, medical history, and present status and then executes appropriate follow-up. Handles all prescription refills and call-in to pharmacies and patients.# Review patient messages to assess clinical status, coordinate care with providers and schedule appointments according to established protocol.# Administers immunizations and injections as directed by the primary care provider.# Communicates laboratory and other test results to the patient as directed by the primary care provider. #Possesses a willingness to adapt to changing department demands, including assisting clinical staff with procedures, such as EKG, cleaning wounds, obtaining specimens and cultures according to HMG policy. Demonstrates willingness to assist co-workers during heightened patient care activity and serves as a resource for clinical staff. Performs other duties as assigned. Qualifications: Registered Nurse: Graduate of an accredited program for Registered Nurses; current license as a Registered Nurse within the Commonwealth of Massachusetts; current CPR; two years of experience in a primary care provider#s office. Licensed Practical Nurse: Graduate of an accredited program for Licensed Practical Nurses; current license as a Licensed Practical Nurse within the Commonwealth of Massachusetts; current CPR; two years of experience in a primary care provider#s office.
