Job Summary:
Under supervision of the Sr Manager of Clinical Pharmacy Services or Sr Manager, Ambulatory Pharmacy Services, as designated, assists with the development, implementation and maintenance of educational programs to influence prescribing practices of practitioners to achieve optimal effectiveness and cost efficient high quality pharmacotherapy, provide direct patient care related to Oncology and supportive drug therapy, identify and prioritize opportunities to improve quality and cost effectiveness, reduce physician prescribing practice variations and provide drug information. Performs direct patient care duties and other regional administrative duties. Serves as scholarly expert within the Oncology service area. Provides clinical pharmacy services that advance optimal patient care, appropriate drug use management, education, research and patient safety. Reviews chemotherapy protocols and utilizes a comprehensive electronic medical record to assist with appropriate drug therapy selection, management, monitoring and education. You will serve as an Oncology pharmacy expert.

Essential Responsibilities:


  • Specific patient care responsibilities may include: Assessing medication regimens, obtaining medication histories, enrolling patients into clinical service area, counseling and educating patients and/or caregivers regarding drug therapy and disease management (at office visit or phone visit), coordinating drug therapy with home health care services and/or practitioners, notifying patients regarding dosage adjustments, scheduling lab follow-up appointments, designing appropriate drug treatment plans for patients, compliance with TSPMG practice guidelines and KPGA drug therapy management protocols, managing adverse events and drug interactions, using computerized pharmacy databases for patient tracking and documentation, and generating consultations to practitioners or notes for medical record entry.

  • Provides pharmaceutical care for members including prospective, concurrent and retrospective drug utilization review, screening for drug allergies, monitoring interactions and duplications, following-up with physicians as appropriate, and counseling patients. Interacts and consults with practitioners when necessary to improve quality of care and improve value; educates practitioners regarding cost effective therapy and drug utilization goals (e.g. preferred drug, formulary and drug promotion).

  • Develops and/or maintains a Oncology clinical pharmacy practice site services, including chemotherapy selection, drug therapy monitoring and patient counseling, as appropriate.

  • Works in conjunction with Oncologists, Infusion Pharmacists and Nursing staff to optimize chemotherapy and related supportive medication drug selection, ordering, preparation and administration.

  • Assists practitioners and administration to ensure high quality of care in specific pharmacotherapeutic areas, e.g. those related to HEDIS measures, clinic services and initiatives.

  • Monitors physician drug utilization practices to identify, implement and evaluate effective techniques to favorably influence prescribing (e.g. target drug programs, practice variation surveillance, consistent reporting to physicians).

  • Provides drug information to health care practitioners, pharmacists, medical office staff and members. Leveraging pertinent data to influence prescribing practice and patient intervention.

  • Develops and presents continuing education programs and staff development in-services for practitioners, pharmacists and other medical office staff.

  • Assists with development, monitoring, and reporting of medication usage evaluation (MUE) criteria when necessary.

  • Assists with surveillance and reporting of adverse drug reactions.

  • Tracks and reports quality improvement activities related to clinical pharmacy services periodically to appropriate manager.

  • Apprises management regarding drug education and promotional program goals, activities and progress.

  • Develops and participates in oncology clinical trial research and scholarly activity, depending on the scope and availability of the service.

  • Assists in the educational and professional development of pharmacy students and residents.

  • Comprehend and utilize all policies and procedures and standing orders for the Pharmacy Department.

  • Provides additional support to the Pharmacy Department as business needs dictate or as otherwise assigned.

  • Develops and/or maintains and adheres to clinical practice standing orders, drug therapy management protocols and guidelines as required.

  • Provides training and mentoring to new and current clinical staff in areas of expertise. Serves as a resource to existing staff.

  • Performs scholarly research as necessary to ensure clinical activities within the service area are in accordance with current practice standards, and serves as scholarly expert to existing clinical staff.

  • Maintains active membership and participation in Kaiser Permanente multi-disciplinary committees and work groups related to Oncology Clinical Pharmacy practice.

  • Provides disease statement management and participates in all aspects of the management of patients on oral chemotherapy agents.

  • Participates in all aspects of the medication use system related to the use of antineoplastic agents while providing comprehensive and individualized pharmaceutical care.

  • Maintain BCOP certification, if required below per Minimum Education Requirements.
