 Remote Medical Record Chart Reviewer
 
Workplace: Remote- Full Time OR Part Time
Length: 6 month contract
Schedule: Training will be on Mon-Fri 8am-5pm PST
Core Schedule: Flexible
 
 
Overview
We are seeking a detail-oriented and skilled Medical Record Chart Reviewer to join our healthcare operations team. This position ensures accurate clinical documentation and directly supports timely patient care. The ideal candidate brings a strong foundation in medical terminology, healthcare systems, and regulatory standards, with a passion for improving patient outcomes through excellence in data management.
 
Responsibilities

  • Navigate and review digital patient charts using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
  • Apply domain-specific knowledge in oncology, hematology, and pathology to support specialized workflows.
  • Interpret and analyze medical terms, conditions, and abbreviations to ensure accurate data capture and reporting.
  • Evaluate clinical notes, reports, and healthcare documentation for completeness and clarity.
  • Ensure adherence to healthcare regulations, including HIPAA, to maintain patient confidentiality.
  • Identify inconsistencies, errors, and missing information in clinical records and escalate as needed.
  • Analyze complex datasets to identify trends and improve operational efficiency.
  • Maintain organized records and manage multiple tasks with effective prioritization.
  • Document findings clearly and communicate with internal teams.
  • Operate independently in remote or specialized roles, managing tasks with minimal supervision.
  • Perform other related duties as assigned.
 
Qualifications
Required:
  • Bachelors in Nursing, Biology, or related field
  • Background in Health Information Management, Nursing, or a related clinical field.
  • Proficiency in EHR/EMR systems and familiarity with clinical documentation standards.
  • Knowledge of HIPAA and other relevant healthcare regulations.
  • Exceptional attention to detail and strong analytical skills.
  • Excellent written and verbal communication skills for timely information transfer within teams.  
    Preferred:
    • Prior experience in a Medical Records Chart Reviewer role.
    • Experience in oncology, hematology, or pathology.
