Nurse Practitioner, MedStar Health Diabetes Boot Camp (Remote)

We are seeking an experienced Nurse Practitioner to join the MedStar Health Diabetes Boot Camp, an established telehealth program that provides intensive, evidence-based diabetes management and education to individuals living with diabetes. This role offers a unique opportunity to develop ongoing therapeutic relationships with patients through frequent virtual visits, helping them overcome barriers, build confidence, and achieve meaningful improvements in their health.

 

As a member of our multidisciplinary team, you will work closely with registered dietitians, diabetes care and education specialists, support staff, and physician leaders to deliver coordinated, patient-centered care. You will manage patients through a structured 14-week program, conducting weekly follow-up visits, adjusting medications, interpreting glucose data, providing education, and partnering with patients to develop personalized care plans that fit their individual goals, lifestyles, and preferences.

 

The Nurse Practitioner in this role will work remotely from home, assessing, diagnosing, treating, and educating individuals who are newly diagnosed with diabetes or those with established diabetes requiring additional support and intensive management. Candidates should have a minimum of 1-2 years of diabetes management experience as an advanced practice provider. Experience with continuous glucose monitoring (CGM), insulin pumps, and automated insulin delivery (AID) systems is highly desirable. CDCES and/or BC-ADM certification is preferred but not required.

  • A collaborative approach and ability to work effectively within a close-knit multidisciplinary team
  • A commitment to delivering exceptional patient experiences and outcomes

Schedule

  • Monday through Friday
  • 9:00 a.m. to 5:30 p.m.
  • No weekends or holidays

This position has a hiring range of

USD $115,003.00 - USD $189,999.00 /Yr.
