MedStar Health is looking for a Chief Social Worker MHCP to join our team at MedStar House Call Program!

The Chief Social Worker, MHCP, leads the social worker team in providing care coordination, psychosocial support, and community outreach to complement the multidisciplinary team approach for primary health care to home bound patients. Responsible for monitoring all Total Elder Care social workers.

Primary Duties:

· Audit & Compliance Support Assists the AVP of Operations with external audits and ensures adherence to governing regulations, policies, and procedures.

· Clinical Oversight & Program Development – Creates, enforces, and refines clinical policies while designing innovative care delivery models to enhance MedStar Total Elder Care.

· Staffing & Team Management – Determines staffing levels, manages caseloads, recruits and mentor's social workers, and monitors clinical competencies to maintain high-quality care.

· Patient Care & Support – Carries a limited caseload, provides direct clinical services, and supports the Social Work team in delivering patient-centered care consistent with MedStar Health's mission and philosophy.

· Quality & Community Engagement – Contributes to departmental goals, participates in quality improvement initiatives, and represents the department through committees and community outreach.

Qualifications:

· Master's degree in social work. (Required)

· 3-4 years post grad experience as a social worker, home care experience (Required)

License/Certifications:

· LICSW - Licensed Independent Clinical Social Worker Licensed Independent Clinical Social Worker Upon Hire (Required)

This position has a hiring range of $72,758 - $130,041
