MedStar Health is looking for a Clinical Social Worker to join our team at MedStar Washington Hospital Center!  

The Clinical Social Worker supports MedStar Washington's resource management mission through clinical discharge planning activities that help ensure appropriate bed utilization and length of stays. Provides psychosocial assessment of patients/families and significant others to facilitate a safe discharge plan. Provides direct clinical counseling services to patients, families, and groups. Identifies appropriate community resources and makes referrals. May specialize in a particular area, such as AIDS or OB/GYN.

Primary Duties: 

  • Interviews patients/family and/or significant others to evaluate medical, psychosocial, financial, and environmental barriers to a safe and timely discharge. Formulates an accurate assessment and develops an intervention plan consistent with the data received during the evaluation.  

  • Engages in multi-systems management to resolve all system influences that present potential barriers to the safe, timely discharge of the patient. Identifies and maximizes the use of available psychological, environmental, and health care resources to assist in barrier elimination.  

  • Assists patient/family and/or significant others through individual, family, and group intervention to resolve psychological barriers that impede optimum social and health adjustment. Maximizes the patient/family ability to understand, accept, and comply with medical recommendations.  

  • Documents all assessments, recommendations, and clinical discharge planning activities in the medical record. Contributes to the department's training, educational, and consulting activities. Utilizes resources, such as family and community-based agencies, to assist patients to resume life in community or to learn to adjust to disability. Recommends and processes applications for other home placement, such as nursing homes, hospices, treatment centers, etc.  

  • Participates in improving health services and quality control mechanisms, through close collaboration with physicians and other health team members. Adheres to departmental, MedStar Washington, and National Association of Social Workers (NASW) policies, protocols, and professional standards. 

 

Qualifications:  

  • Master's degree in social work. 

  • 1-2 years of progressively more responsible job-related experience. 

  • Licensed Social Worker in the District of Columbia.
