General Summary of Position

Medstar Health is looking for a Clinical Social Worker to join our team on the Community Pediatrics Mobile Van! We are looking for a professional clinical social worker with knowledge and practice skills related to psychosocial aspects of illness, knowledge of community resources, excellent verbal and written communication skills, and effective interdisciplinary/ interpersonal skills for practice and consultation.

The Clinical Social Worker will provide direct services to patients and families, social work consultation, leadership and teaching functions for staff, nurses, and providers, and patients. Adheres to National Association of Social Workers (NASW) Code of Ethics and exemplifies the qualities and values of the social work profession.

Primary Duties:

  • Provides clinical social work services to identified patients and families including individual and group counseling regarding the impact of illness and hospitalization. Serves as a resource for patients with psycho-social issues and provides a link to psychiatric resources. Assists patients dealing with chronic pain, pain medication management and substance abuse problems. Leads support groups for patients and families if licensed permits.

  • Collaborates with other health care professionals. Facilitates communication regarding relevant psycho-social issues of patients and families to health care teams through recording in medical charts, consultation and participating in meetings. Represents social work perspective and serves as department representative to designated Hospital committees and community organizations. Participates in training and educational programs for students, physicians, nurses, and other health care professionals. Serves as a resource for providers, nurses, and staff for effectively managing difficult patients. Provides support and training to triage nurses handling calls from anxious and suicidal patients.

  • Functions as a coordinating link between patient, family, hospital, and community resources to assure continuity of care. Participates in meetings and on committees and represents the department and hospital in community outreach efforts. Participates in multi-disciplinary quality and service improvement teams. Coordinates community resources for underinsured patient populations. Works with the VNA and assists with establishment of home healthcare. Proactively coordinates community resources for uninsured and underinsured patient populations.

  • Assists in identifying programs and manufacturers to cover costs of medications, including chronic and long-term pain medications. Helps with patients with compliance issues, such as medication problems or missed appointments due to transportation issues. Tracks and reports outcomes from social work intervention. 

  • Assumes responsibilities for identifying training needs, locating resources, and promoting their own professional development.

Qualifications:

  • Master's degree in social work from an approved Council on Social Work Education program.

  • Current Licensed Independent Clinical Social Worker (LICSW) license in the District of Columbia, Maryland, or Virginia based on work location(s).
