Unit Highlights
We are seeking a compassionate and knowledgeable Clinician to serve as a key resource for our nursing associates, physicians, and patients in providing glycemic and diabetes education for inpatients. As the coordinator of the Glycemic Control Program, you will play an essential role in educating, supporting, and guiding patients and their families, while working closely with the multidisciplinary team to ensure optimal patient outcomes. The patients served range from infancy through adulthood.

Key Responsibilities:

  • Act as a knowledgeable resource for nursing staff, physicians, and patients regarding diabetes management and glycemic control. Provide ongoing education, emotional support, and psychological assistance to patients and families.
  • Lead the program by staying informed about the latest practices and theories in glycemic care and diabetes management. Share this knowledge with the multidisciplinary team, consulting with physicians and staff about diagnostic tests, medication management, and treatment adjustments based on individual patient needs.
  • Evaluate and treat diabetic patients using appropriate assessment tools. Assess patients with abnormal glucose values, reviewing their medical history, medication records, blood pressure, lab values, and overall health status.
  • Develop and deliver educational plans to the multidisciplinary team, identifying learning needs and providing mentoring as necessary. Implement adult teaching strategies and follow up to evaluate educational outcomes.
  • Provide tailored educational sessions to patients and their families, ensuring their emotional and psychological needs are addressed. Support patients in understanding their medications and developing strategies for adherence.
  • Recommend wellness and prevention programs for patients, caregivers, and family members to promote long-term health and glycemic control.

Qualifications

  • BSN, MSN, or CNL from an accredited School of Nursing required.
  • Minimum of three years of nursing experience
  • Active MD RN License or Active Compact State RN.  Certified Diabetes Educator Certification preferred or obtained within 12 months.
