General Summary of Position
We are actively recruiting for a Medication Safety Pharmacy Specialist to join our team at MedStar Montgomery Medical Center in Olney, MD. This position includes medication safety plus frontline staffing in the capacity of a clinical pharmacist. Shifts include days/evenings/nights and rotating weekends: 7a-3:30p, 8:30a-5p, 9:30a-6p, 11:30a-8p, 8p-7a.
Manages and coordinates projects relative to patient medication safety and/or the reduction in adverse medical/healthcare events. Identifies opportunities to enhance safety utilizing internal or external data, develops action plans and related goals, educates and trains, enhances an institutional culture of safety, and measures outcomes to determine effectiveness.
Primary Duties and Responsibilities
Assists in the evaluation of the medication safety program as measured against external accrediting organization standards (i.e. TJC, NPSG's). Communicates the state of medication safety in the institution on a periodic basis. This would include preparation of a Medication Occurrence Annual Report, and regular updates to the Quality Council. Develops and performs internal audits, as necessary, to determine the compliance and effectiveness of policies and processes. Provides periodic reports to the appropriate department(s) and/or discipline(s). Develops educational programs and assists in training efforts to improve medication safety. Utilizes a multidisciplinary, collaborative approach to foster a culture of safety in the institution. Ensures compliance with internal and external reporting requirements for actual and potential adverse drug events and/or medication occurrences using the Medication Occurrence Reporting System, USP, FDA, ISMP, etc.Evaluates the literature for new technologies or successful practices that have been demonstrated to enhance safety in other organizations and share this information via interdisciplinary team education. Manages the hospital Medication Occurrence Reporting System (MOS.) Ensures for the complete and consistent investigation of all reported medication occurrences. Evaluates and analyzes the reported occurrences to identify trends and opportunities to improve medication safety. Participates in MOS User Groups to maintain competence and provide feedback for system improvements. Participates in multidisciplinary quality and service improvement teams. Participates in the in-depth analysis and evaluation of the medication management system and/or related processes both prospectively and retrospectively. Assists in the development of action plans and recommendations to help implement risk reduction strategies. Performs duties of the Patient Care Pharmacist to ensure prompt and accurate dispensing of all inpatient medications. Provides input and assists in the development of hospital goals, objectives, policies and procedures relative to medication safety. CUSTOM.PRIMARY.DUTIES.RESPONSIBILITIES.ADDENDUM
Minimum Qualifications
Education
- Doctoral degree in Pharmacy required
- PGY2 Medication-Use Safety and Policy pharmacy residency preferred
Experience
- 5 years clinical experience as a practicing pharmacist in an institutional setting required or previous experience in management of pharmacy safety and compliance required
Licenses and Certifications
- PHARMD - Licensed Pharmacist Maryland Pharmacy license required
Knowledge, Skills, and Abilities
- Ability to communicate effectively with all internal and external customers.
- Moderate Computer skills to perform prescription entries.
- Experience with computerized Pharmacy information systems § HIPAA policies on Confidentiality
This position has a hiring range of $118,331 - $230,172
