Description

The Transfer Center Nurse Navigator is responsible for coordinating patient transfers and ensuring the seamless transition of care from one hospital to another. This role involves managing the transfer process from initial request to final placement, facilitating communication between healthcare providers, and advocating for the best possible patient outcomes. These functions are performed in accordance with all applicable laws and regulations and MedStar Health's philosophy, policies, procedures, and standards.
Education

  • Bachelor's degree in Nursing, from an accredited School of Nursing required
Experience
  • 5-7 years Experience in a clinical nursing setting. required and
  • 3-4 years Experience in critical care setting. preferred
Licenses and Certifications
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure State of Maryland or compact.and the District of Columbia Upon Hire required
Knowledge, Skills, and Abilities
  • Computer skills with word processing abilities required.
  • Must possess strong clinical assessment good skills with knowledge of the nursing process.
  • Effectively prioritize responsibilities, manage multiple tasks simultaneously, and consistently maintain courteous interactions with all callers, professionals, and staff members
  • Must have excellent interpersonal skills and telephone manner.
  • Coordinates and manages interfacility patient transfers, ensuring timely and efficient placement of appropriate patients in appropriate care settings
  • Tracks repatriation requests to help facilitate system load balancing.
  • Communicates effectively with referring and receiving facilities to gather necessary patient information and provide updates on transfer status
  • Collaborates with physicians, nursing staff, and other healthcare providers to assess patient needs and determine the most appropriate care setting.
  • Monitors and document patient transfer status, including any significant changes, delays, or issues, and communicate these to relevant parties.
  • Serves as a liaison between referring physicians, patients, and hospital staff to ensure a smooth and efficient transfer process.
  • Advocates for patient safety and quality care throughout the transfer process.
  • Maintains accurate and up-to-date documentation of all transfer activities and communications in the electronic transfer record.
  • Participates in quality improvement initiatives and ongoing development and implementation of policies and procedures related to patient transfers.
  • Maintains organization, and availability of resources, protocols, and equipment necessary for daily operations. Troubleshoots equipment when necessary.
  • Exemplifies guest and staff relations standards in all activities. Abide by and promote the MedStar Mission, Vision and Values.
