Mercy is seeking a dedicated and experienced Clinical Pharmacist – Hospital to join our pharmacy team. This role works collaboratively with healthcare professionals to provide safe and effective drug therapy, serving as a trusted drug information resource for pharmacy, nursing, medical staff, and patients. The Clinical Pharmacist performs all duties in alignment with Mercy's mission, values, and Service Standards.
Key Responsibilities: Primary Aspects:- Review patient records to assess appropriateness of medication therapy.
- Evaluate patient conditions to ensure comprehensive treatment.
- Develop medication plans that minimize adverse effects.
- Consult with prescribers, nurses, and patients on medications and dosages.
- Advise on proper drug administration and monitor treatment outcomes.
- Maintain accurate documentation of medication plans and patient progress.
- Routinely consult with healthcare professionals regarding medication orders and care path management.
- Stay informed on current policies and procedures.
- Maintain strong professional relationships with medical and nursing staff.
- Attend conferences and maintain licensure requirements.
- Serve as a clinical resource for care center staff and practice Medication Therapy Services.
- Participate in committees and comply with regulatory standards.
- Support drug utilization evaluations and adverse drug reaction reporting.
- Educate students and residents and participate in departmental initiatives.
- Evaluate medication orders for clinical and operational concerns.
- Participate in investigational protocols as needed.
- Prioritize daily activities and demonstrate fiscal responsibility.
- Present in-services and lectures to support continuing education.
- Assist in developing staff competencies related to patient care.
- Participate in quality assurance activities and recommend resolutions.
- Monitor drug therapy and intervene when appropriate.
- Coordinate efforts with operational staff.
- Precept undergraduate B.S. and Pharm.D. students.
- Adhere to confidentiality policies and procedures.
- Complete expiration date checks and inspections.
- Perform pharmacy and unit floor stock checks.
- Complete mandatory fire/safety training and continuing education.
- Use pharmacy information systems (e.g., Epic) for order verification and documentation.
- Utilize drug information software (e.g., Micromedex).
- Operate pharmacy automation systems (e.g., Omnicell, ScriptPro, Baxa, McKesson).
- Demonstrate Mercy Signature Service communication skills.
- Maintain professional and courteous behavior.
- Follow proper hygiene, PPE, and dress code standards.
- Work efficiently under time constraints and meet hourly deadlines.
Education:
- Required: Bachelor of Science (BS) or Doctor of Pharmacy (PharmD) from an accredited school of pharmacy.
- Preferred: Doctor of Pharmacy (PharmD)
Licensure:
- Required: Current pharmacist license in the state of practice.
Experience:
- Required: Minimum 1–4 years of experience in area of practice.
- Preferred: Two or more years of clinical experience and completion of a PGY-1 residency.
