This role is primarily telephonic. The role will be remote once training is complete.
At Rooney Breast Center and Surgery Clinic, our goal is to detect breast cancer at its earliest possible stage to maximize treatment options and minimize disease-burden. Providing a risk assessment to our patients at their annual mammogram encounter will allow us to provide supplemental screening for women with a higher risk for breast cancer. The High-Risk Nurse Navigator will help to manage women screened for additional breast cancer risk. Following a collaborative team model, the High-Risk Nurse Navigator identifies, supports, and manages this patient population through the delivery of tailored and individualized care.
The High-Risk Nurse Navigator at the Sindelar Cancer Center is responsible for guiding a patient through the comprehensive high-risk for breast cancer treatment plan designed for them as a whole person. This role includes education, support, coordination, and advocacy, for the patient and family. The goals of this position include ensuring high quality patient care, easy access to the care needed, barrier elimination, and excellent patient experience by connecting the patient and family with appropriate resources and services. The High-Risk Nurse Navigator will treat all patients entrusted in their care with dignity, showing the love and compassion of Jesus.
REQUIREMENTS:
- Experience: Required 2-3 years' experience in nursing
- Preferred Experience: 3 years' experience in outpatient setting (strongly preferred experience in oncology, radiology, and/or breast surgery clinic)
- Required Education: Graduate from accredited school of nursing (Registered Nurse) with active Missouri RN license
- Required Degree: Bachelor's Degree (preferred advanced certifications in oncology and/or nurse navigation)
- Certifications: Active BLS (Basic Life Support) certification required
- Preferred Certification: OCN, CGRA, BHCN, CN-BN, or other related certifications
- Preferred Other: Public speaking skills, easily understands and integrates new information, experienced with evidence based best practice and protocol development/improvement.
RESPONSIBILITIES:
- Engage with patients in a telephonic format for approximately 85% of this role; 15% of this role will be through electronic communication via the EMR (patient portal messaging)
- Provides telephonic formal risk assessment for women initially screened as high-risk for breast cancer to ensure accuracy of breast cancer screening recommendations
- Gain a rich knowledge and understanding of high-risk screening guidelines, recommendations, and follow-up measures to serve as a subject matter expert
- Assist with patient scheduling for breast specialist clinic and high-risk screening, to ensure annual follow-up
- Create, plan, and implement evidenced-based measures for high-risk women that ensures compliance with all regulatory guidelines
- Provide necessary support and care navigation to patients through the high-risk care continuum to minimize care caps and maximize access to screening
- Assist in the development of evidence-based patient care maps, algorithms, and personalized care plans for high-risk patients
- Collect, analyze, and report monthly data to ensure high-quality, evidence-based care is delivered to this at-risk patient population
- Serves as an ongoing resource for patients and families throughout the high-risk care continuum for screening and surveillance.
- Develop and implement education-based initiatives and materials for the referring provider community to improve access to risk screening and surveillance
- Collaborate with referring providers and other multidisciplinary team members to ensure maintenance of high-risk screening recommendations
