Mercy Hospital is seeking a dedicated and compassionate Nurse Practitioner or Physician Assistant to join our innovative Hospital Care at Home (HCaH) team. We're seeking a growth-minded, results-oriented individual with high energy and strong interpersonal skills. Hospital Care at Home

 

This Advanced Practice Provider will serve as a key driver of admission volume and internal referral growth for the Hospital Care at Home service. The APP will lead the identification, acquisition, and transfer process of eligible patients from hospital to home, ensuring seamless, high-quality transitions of care. Minimum of two years of inpatient clinical experience as an Advanced Practice Provider required.

  • Responsibilities include patient screening, care coordination, clinical management, patient education & consent, service growth & promotion, & quality improvement
  • 7 on/7 off day shift schedule
