Under the direction of the primary care operations team, the Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel. Position Details:

Mercy Clinic 65 Prime Plus - North County 637 Dunn

Schedule: Monday - Friday, 0800-1630

No weekends or holidays!

Mercy 65 Prime+ Clinics are a new primary care service designed specifically for Medicare beneficiaries. Patients receive extended office visit times and closer monitoring of chronic conditions like high blood pressure, diabetes, and heart disease. As a Primary Care Nurse (PCN) in this clinic, you will work closely with the providers to triage incoming calls/messages, educate patients, follow-up with patients, and assist with various clinic tasks as needed.

 

LPN Qualifications:

  • Education:  Diploma in Practical Nursing

  • Licensure: Active LPN license

RN Qualifications:

  • Education: Associate of Applied Science in Registered Nursing, Graduation from registered nursing program, bachelor's degree in nursing (BSN), OR 4 years of commensurate experience in lieu of the bachelor's degree

  • Licensure: Active RN license 

Highly Preferred Skills:

  • Proficiency with Epic EMR system.

  • Experience working with geriatric populations.

  • Timely and thorough patient triage skills.

  • Computer competency (most of this position will be computer work).

  • Excellent time management and prioritization.

  • Adaptability in a new and changing environment.
