Under the direction of the primary care operations team, the Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel. Duties and responsibilities will be performed in a manner consistent with our mission, values, and Mercy Service Standards. Position Details:

Mercy Clinic 65 Prime Plus - North County

637 Dunn Road

Hazelwood, MO 63042

Schedule: Monday – Friday, 0800-1630

No nights, weekends, or holidays!

Must complete 90-days of on-site training prior to transitioning to hybrid work-from-home schedule - One (1) day per week.

Mercy 65 Prime+ clinics are a new primary care service designed specifically for Medicare beneficiaries. Patients receive extended office visit times and closer monitoring of chronic conditions like high blood pressure, diabetes, and heart disease. As a Primary Care Nurse (PCN) in this clinic, you will work closely with the providers to triage incoming calls/messages, educate patients, follow-up with patients, and assist with various clinic tasks as needed

LPN Qualifications:

Education: Diploma in Practical Nursing

Licensure: Active LPN license

Experience: 2-3 years of experience preferred

RN Qualifications:

Education: Associate of Applied Science in Registered Nursing, Graduation from registered nursing program, bachelor's degree in nursing (BSN), OR 4 years of commensurate experience in lieu of the bachelor's degree

Licensure: Active RN license

Experience: 2-3 years of experience preferred

Highly Preferred Skills:

Proficiency with Epic EMR system

Experience working with geriatric populations

Timely and thorough patient triage skills

Computer competency (most of this position will be computer work)

Excellent time management and prioritization

Adaptability in a new and changing environment
