Job Description:
Under the leadership of the department Director, Assistant Directors, and/or Pharmacy Supervisors, the Clinical Pharmacist is responsible and accountable for planning, directing, assessing and providing pharmaceutical care according to the policies and procedures of the hospital and department. Important dimensions of this position consist of quality of care, professional commitment, teamwork, interpersonal skills, safety, customer service, fiscal responsibility, and a broad scope of pharmacy knowledge. This role requires ongoing professional development, including attending continuing education conferences, remaining up to date in pharmacy literature, and presenting patient cases or doing medication reviews. The Clinical Pharmacist manages pharmacy protocols, provides inpatient medication order screening and verification, patient consultations, and completes job functions and documentation timely. Responsibilities include patient assessment and medication management including order processing, sterile compounding, reviewing patient medication regimens, therapeutic drug monitoring, competently prescribing per drug protocols, providing drug information, preventing, identifying and reporting adverse drug reactions and medication errors, giving patient consultation, development of plans of care, and the oversight of Pharmacy Technicians. The Clinical Pharmacist also responds to various emergency situations including codes. The Clinical Pharmacist will be able to cross-cover the Inpatient Pharmacist when needed.
Experience
Candidates are required to have at least one of the following:
- Completed an accredited PGY1 Pharmacy Residency (which is the preferred option), or
- Worked at least three years in a Clinical Pharmacist position within the prior 5 years, or
- Worked at least five years as an Inpatient Pharmacist or Acute-care Pharmacist.
Education
As required to meet State of California licensure requirements.
Must pass orientation and initial competency assessment prior to independent assignment of the applicable skill, and must complete and pass all annual competencies.
Licensure/Certifications
State of California RPH (Pharmacist) license is required. American Heart Association Healthcare Provider BLS is required. ACLS must be completed prior to completion of the introductory period and independent assignment. Board certification preferred. DEA license required and must be completed at the end of the introductory period.
