Job Description:

The Palliative Medicine Service Clinical Social Worker reports to the Palliative Medicine Service Manager and works collaboratively with the Interdisciplinary Palliative Team as well as patient's larger health care team. Caseload can consist of inpatients, outpatients, and their families.  The Palliative Social Worker is responsible for completing thorough psychosocial assessments with patients with serious illnesses and their family members.  Based on assessment, the Palliative Social Worker develops treatment plans, provides supportive counseling, crisis intervention, and makes referrals to appropriate community resources.  Social work services include assisting patients and their families in understanding and coping with the emotional and social problems related to their health status.

Experience

Requirements:

Two or more years of experience in social work post master degree.   Ability to effectively interact with patients and significant others during times of transition of care, complex, and crisis patient and family situations. Effective communication and public relation skills. Ability to work as an integral member of an interdisciplinary team.

Preferences:

Two or more years of experience in palliative care, oncology, and/or hospice.  One year experience in home health. Knowledge and experience with advanced health care directives and POLST. Knowledge and experience with bereavement care, working with grieving individuals and families. Knowledge of local community resources.

Education

Master's degree in Social Work.

Licensure/Certifications

Current license to practice in the State of California (LCSW). American Heart Association Healthcare Provider BLS required. Must possess a valid California driver's license.

This position may require driving. In addition to a valid CA driver's license, the following are required: Automobile insurance as required by the State of California with an acceptable driving record of three points or less, participation in the DMV pull-notice program, and an automobile available for daily work.

The hours employees work determine when a shift differential is paid.

Hourly Evening Shift Differential: $3.39 Hourly Night Shift Differential: $5.09
