Job Description:

The Nurse Practitioner, Ostomy and Wound Program reports directly to the Assistant Director, Wound Program and under the supervision of the program Medical Director. S/he is responsible for providing wound care and ostomy management and education to inpatients and outpatients.

The Nurse Practitioner, Wound Program possesses a broad scope of knowledge and skills in ostomy and wound care to provide patient care and facilitate patient self-care management. Communicates with and collaborates with physicians, other hospital nursing staff, and other healthcare team members to provide consistent quality care for ostomy and wound program patients. The ostomy and wound program Nurse Practitioner practices as a member of the interdisciplinary team.

The Nurse Practitioner works with a high degree of autonomy and is responsible and accountable for assigned patients as a case manager to plan, evaluate, and implement the care for patients within the framework of the State Nursing Practice Act. The Nurse Practitioner complies with all hospital policies and procedures while performing job duties and responsibilities including, but not limited to: participation in the program's daily operations; direct patient care; and providing education to patients and families. Educates, coaches and assists nursing team members to ensure optimal wound and ostomy care and efficient use of resources and documentation. Integrates knowledge of age and developmental specific care, institutional goals and standards, current best practice standards. Uses standardized procedures to independently examine, diagnose, and treat patients in collaboration with supervising physicians

Experience

Three to five years acute ostomy or wound care RN nursing experience in an acute care hospital setting. Ability to cultivate relationships within all departments, exhibit integrity and initiative, and is collaborative and professional. Ability to prioritize easily and quickly. Communicates with discretion, confidently and effectively with superiors, peers, and subordinates.

Preferences: Exhibits genuine interest in, enthusiasm and passion for, and dedication to ostomy and wound care.  Keeps up-to-date with the latest evidence-based practice. 

Education

Master's Degree in Nursing required- Exception Nurse Practitioners' qualified or certified as an NP in California or any other state prior to January 1, 2008.  Graduate of an approved Nurse Practitioner program.  National certification in ostomy as a Family Nurse Practitioner. 

Licensure/Certifications

Must be currently licensed in California as an NP and be certified as a nurse practitioner by a recognized national certifying body that has established standards for nurse practitioners to practice in the State of California. State of California RN license required. Furnishing license issued by the Board of Registered Nursing required. Certification from either ANCC or AANP as a nurse practitioner. American Heart Association Healthcare Provider BLS certification required.

Advanced wound certification (e.g. Certified Wound Specialist (CWS) and/or Wound, Ostomy, and Continence Nursing (WOCN) certification preferred. Active DEA number for Schedule II-V drugs optional..

The hours employees work determine when a shift differential is paid.

Hourly Evening Shift Differential: $5.41 Hourly Night Shift Differential: $8.11
