At NorthBay Health, the Licensed Clinical Social Worker (LCSW) Mentor covers both NorthBay and VacaValley hospitals Social Work team. The LCSW Mentor works under the supervision of the Manager of Inpatient Case Management/Social Services. A LCSW Mentor provides guidance, supervision, and professional development support to social workers, interns, and other mental health professionals. This role focuses on enhancing clinical skills, ensuring ethical practice, and promoting evidence-based interventions. The LCSW Mentor offers case consultation, facilitates reflective learning, and supports mentees in navigating complex client situations while adhering to regulatory and organizational standards.

Furthermore, the LCSW Mentor may contribute to training programs, lead workshops, and assist in evaluating clinical competencies. They foster a supportive learning environment that encourages growth, cultural humility, and professional accountability. Strong communication, leadership, and clinical expertise are essential, along with a commitment to advancing quality care and outcomes for diverse populations.

In addition to mentoring responsibilities, the LCSW Mentor provides direct clinical social work service, counseling, and discharge planning services to hospital patients and their families including biopsychosocial assessment and documentation of emotional, psychological, social, financial, environmental or follow-up referral needs.  Ability to initiate and discontinue 5150 custodial holds WITHIN NorthBay Medical Center and VacaValley Emergency Rooms, and inpatient units. (5150 Custodial Hold: Welfare and Institutions Code section 5150 et seq. specifies the circumstances under which a person who is gravely disabled or a danger to themselves or others as a result of a mental illness may be detained against their will for psychiatric evaluation and treatment for up to 72 hours). 

Manages the psychosocial needs of the patient across the continuum of care by completing a clinical bio-psychosocial assessment, formulating a plan of care in collaboration with the case manager and other members of the multidisciplinary care team. This includes the assessment of patient unique strengths and needs, developing a discharge plan, monitoring and documenting patient progress toward the goals of care, and identifying barriers to achieving an appropriate length of stay.  S/he will work collaboratively with interdisciplinary healthcare staff and facilitate integration with community-based resources to optimize patient health. Communicates appropriate information to care providers in the continuum of care and documents assessment and interventions in the electronic health record.  Activities are consistent with System mission and values and in compliance with applicable regulations and System policies and procedures.  Works under the direction of the Manager of Social Services.



Qualifications

Education

  • Master of Social Work from a school of Social Work accredited by the Council on Social Work Education.

Licensure/Certifications: 

  • California BBS Licensed Clinical Social Worker (LCSW)

Experience: 

  • Minimum of two years' experience within the acute health care setting or the community health setting. 

  • Leadership experience preferred.

  Skills: 

  • Strong clinical assessment skills.

  • Excellent interpersonal and communication skills: verbal, written and listening.  Ability to convey instructions, directions and information appropriately and accurately. Ability to work diplomatically with a wide variety of people. 

  • Strong critical thinking skills- must be capable of independent judgment and have strong clinical decision-making skills. Ability to work independently with minimum direction. 

  • Ability to organize a variety of concurrent activities, set realistic goals and objectives, plan and meet deadlines without compromising quality.

  • Ability to work effectively with diverse populations, including those with respect to age, culture, ethnicity, and gender.

  • Knowledge and understanding of individual, child, and family development, human behavior and communication, the influence of culture and values in social work practice, psychotherapy, case management, and the relation between social work practice and medicine.

  • Skilled at working with patients/families coping with loss and grief.

  • Knowledge of child, elder and dependent adult abuse and domestic violence reporting requirements and other significant legislation and laws regulating social work practice, e.g. Tarasoff, HIPAA, etc. Ability to make appropriate interventions based on law, professional ethics and policy.

  • Knowledge of suicidal behavior, the ability to assess lethality, and the ability to develop appropriate treatment plans.  Knowledge of legal requirements and guidelines for providing psychiatric services including the appropriate use of 5150.

  • Ability to successfully manage conflict. 

  • Basic proficiency with word processing required. Experience with MIDAS Plus, Cerner, and Power Point preferred.

  • Knowledge of acute care, subacute care, long-term care, home care, hospice interventions, rehabilitation options, County mental health services, and other community resources and requirements- strongly preferred.
