Description
At NorthBay Health, the role of the Primary Care Triage Registered Nurse (RN) is to function as a member of the multidisciplinary team and to deliver safe and timely care to patients via telephone or portal using standardized clinical guidelines, resources, and independent nursing judgment. The Triage RN utilizes clinical knowledge and competence, positive communication skills, problem solving and conflict resolution techniques, provide telephone responses and competency in clinical assessment, computer skills and customer service. Ability to effect change, strong skills in assessment, organization and time management.
Assesses or evaluates patients by utilizing patient medical history, triage protocols and physician collaboration. Provides general health information using approved references and is responsible for knowing and supporting regulatory and clinical practices in provider direction and clinical assessment to support the patients in providing high quality care.
The Triage RN works under the direction of the Manager and Director of the Primary Care Call Center. The Triage RN works in collaboration with the physician and his/her respective healthcare teams. The Triage RN works across the healthcare continuum encompassing NorthBay Health, network and out of network services in collaboration with patients/families, physicians, staff and administration to ensure appropriate care at the appropriate level to the member.

Qualifications
Education:
Graduated from an accredited nursing school or college.
BSN is required (excludes those employees grandfathered).
Licensure/Certification:
Current California state RN license required.
AHA or equivalent BLS required.
Experience:
One year inpatient or equivalent nursing experience required. Relevant outpatient clinical experience preferred.
Minimum of one year case management experience preferred; equivalent case management education will be considered.
Knowledge of: CMS, TJC and EMTALA standards and federal and state regulations. Knowledge of telephone-based clinical assessment techniques. Current NCQA and HEDIS quality standards, trends in health care, best practices, management tools, and familiarity with related community resources and literature.
Skills:
Computing skills including experience with MS Office (Excel, Word, PowerPoint, etc.) and data using electronic medical records to check patient history and documentation. management.
Ability to effectively communicate with all levels of patients, physicians, health care personnel, supervisory staff, and peers and elicit information needed to make brief evaluations.
Demonstrates the exceptional ability to be organized and efficient in prioritizing and managing assignments with minimal oversight and direction.
Demonstrates the willingness to research, learn, and obtain knowledge for the performance of the position.
Demonstrates a courteous, professional demeanor at all times either working individually or in a team environment working collaboratively in an effective manner maintaining positive relationships with peers and supervisors alike.
Utilizes critical thinking and applies sound clinical judgment and assessment skills for decision making.
Must be able to function in a multi-task environment.
Compensation:
- Hourly Salary Range Min $70.69 - Max $81.83 (Offered hourly rate based on years of experience)
