Description

At NorthBay Health, the Supervisor, Social Services leads the Social Services department. The Supervisor oversees the impact on patient quality of care and system reimbursement through direct oversight of the Hospital and Emergency Department Social Workers, Resource Specialists and Appointment Specialists. The Supervisor is responsible for staffing, department operations and processes.

Primary Job Duties 

  • Functions as a role model.

  • Manages staff scheduling, assignments and daily staff work outcomes.

  • Hold staff accountable for quality work while empowering them to perform at their highest level of work function.

  • Identify opportunities to improve or streamline workflow processes. 

  • Involve and empower staff in operational changes and development of standard work.

  • Identify, develop and implement relevant, valuable staff education and training as needed.

  • Provide regular/frequent staff performance feedback through random staff audits and mentoring to optimize staff productivity. 

  • Prepare and present regular and frequent data reports reflecting department work productivity outcomes and metrics (i.e. readmissions, LOS, avoidable days, documentation compliance, anticipated discharge dates and disposition and white board communication). 

  • Coordinate monthly and quarterly staff meetings to provide department metrics, meaningful information, education and feedback to the team

  • Conduct and delivers annual performance evaluations

  • Keeps staff aligned and helps with progress towards goals. 

  • Monitor and lead ongoing department survey readiness (Joint Commission/ SWS / other). 

  • Provides oversight of social work practice to assure service is in alignment with professional practice standards.

  • Assures appropriate utilization of all available resources to provide quality, cost-effective care.

  • Demonstrates effective problem solving and conflict resolution skills.

  • Manages change in a creative and positive manner.

  • Ensures consistent, positive customer relations throughout each department.

  • Establishes and maintains policies, standards and programs to help assure that the provision of care and services is provided with safety and quality in mind.

  • Ensures adherence to regulatory quality assurance standards by reviewing staff documentation and through direct observation.

  • Collaborate with physicians and hospital staff for care management information or social work concerns. 

  • Oversees staff working to achieve LCSW status to help them obtain their hours.

  • Oversee staff adherence to organizational, professional, state and Federal regulations, guidelines, standards, policies and procedures. 

  • Other duties as assigned.

 



Qualifications

Education

  • Master's degree in Social Work from school accredited by the Council on Social Work Education is required. 

Experience

  • At least 4 years progressive social work experience with complex patients in the inpatient setting, including substance use and mental health crisis patients. 
  • Previous social work leadership experience strongly preferred. 

Licensure/Certification: 

  • Current State of California, Board of Behavioral Science, LCSW required. 
  • Certification in Case Management (CSW or ASW) preferred.  
  • Becomes clinically certified to be qualified to help staff work toward LCSW status.

Skills: 

  • Basic knowledge of personal computer with ability to retrieve data. 
  • Knowledge of MCG, Cerner, Medicare rules and regulations and Conditions of Participation.  
  • Knowledge of Joint Commission, Department of Human Services, Patient Safety, and the Department of Health organizational standards. 
  • Knowledge of acute care, home care, subacute care, long-term care, hospice interventions, rehabilitation options, other community resources and requirements, medical necessity guidelines, quality assurance process and determination of positive outcomes, current standards and trends in health care, best practices, management tools, and familiarity with related resources and literature.

Hours of Work:  

  • 8:00-4:30 p.m. on scheduled days or flexed 8-hour days to accommodate business need.
