- Northwest suburbs – Outpatient clinic located in Crystal Lake, IL with hospital privileges at NM Huntley and NM McHenry hospitals.
- Far west suburbs - Outpatient clinic located in Sycamore, IL with hospital privileges at NM Kishwaukee Hospital. * Visa sponsorship negotiable at this location
