After successful completion of the probationary period, employee will receive off probation wage increase.


Summary:

Provide direct patient-centered care, integrated pharmacy operational services in a decentralized practice setting with health center provider teams, deliver medication therapy management within specialty patient care services, and ensure pharmaceutical care programs are appropriately integrated throughout the division. Participate in all necessary aspects of the medication-use system while providing optimal medication therapy to patients with specific disease states and provide centralized pharmacy support.  


Responsibilities:

* Conduct clinic authorized ambulatory care outpatient pharmacy services following appropriate national and/or local training as well as established facility policies and procedures.
* Design & implement assess, monitor, and document medication-related therapeutic plans utilizing the most effective, least toxic, and most economical pharmaceutical treatments
* Help achieve positive patient -centric goals and outcomes through direct and indirect interactions with patients, providers and interdisciplinary teams.
* Take appropriate health history, conduct and document routine screening such as depression, tobacco, and alcohol as needed, perform limited physical assessment, adjust treatment plans, and monitor outcomes to prescribed treatment plans.
* Interview patients to obtain past medical and medication history, perform chart view to evaluate medication regimens for appropriate drug selections, access for potential drug interactions, therapeutic duplications and noncompliance.
* Document in the EHR all clinical activities, screenings, and update allergy information.
* Order appropriate lab test and interpret results related to medication monitoring as needed.
* Provide Comprehensive Medication Management for complex patients with chronic diseases.
* Review medication profile and may discontinue prescriptions that the patient is no longer using, or the medication has been replaced with a new similar medication.
* Work cooperatively and communicate with providers and clinic administration in the planning and coordination of patient care.
* Attend all Pharmacy & Therapeutic Committee meetings and assist with Prior Authorization requests.
* Supervise students; may train or supervise students as appropriate.
* Staff pharmacist duties based on pharmacy needs
* Prepare various reports as requested.
* Contribute to a team effort and accomplish related results as required

Skills:

* Knowledge of related accreditation and certification requirements

* Ability to relate effectively to patients, patient's families, other members of the health care team
* Knowledge of policies and regulations pertaining to the disbursement of pharmaceutical drugs
* Ability to design and implement systems necessary to collect, maintain, and analyze data

* Ability to establish and maintain professional relationships with individuals of varying social and cultural backgrounds and co-workers at all levels
* Must comply with Caregiver Law and Grant conditions criminal history checks as a condition of employment to include fingerprinting
* Ability to be persuasive and tactful in controversial situations
* Ability to communicate technical information to non-technical personnel
* Operating business computers and office machines, including in a Windows environment, specifically Word, Excel, Access, and presentation software (such as PowerPoint)
* Ability to supervise staff
* Knowledge of drugs and their indications, contraindications, dosing, side effects, and proper administration.
* perform basic patient assessments and referrals.
* Ability to communicate effectively in the English language both verbally and in writing with staff and the general public.

* Basic Life Support (BLS) certification within ninety (90) days of employment
* Obtain Oneida Certification on reporting Child Abuse and Neglect within ninety (90) days.
* Health Insurance Portability and Accountability Act (HIPAA) training is required prior to starting this position and annually thereafter.

Credential equivalent experience:
Essential:
* Licensed as a Registered Pharmacist by the Wisconsin Department of registration and licensing. Must work towards board certification once eligible.

* One (1) year of pharmacy experience in a hospital, clinic, or retail setting.

* Licensed as a Registered Pharmacist by the Wisconsin Department of registration and licensing.

* Must work towards board certification once eligible. 

Nonessential:

* experience with Robotic Dispensing system

Must be willing and able to obtain additional related education and training.

Must pass a pre-employment drug screen and adhere to the Nation Drug and Alcohol-Free Workplace Policy.

A Tuberculosis (TB) Screening and/or a 2 step TB Skin Test is required within thirty (30) days of employment.

Must provide a copy of diploma/degree, license, or certification upon employment.
