Schedule: Monday–Friday, 8 a.m.–4:30 p.m. - Full Time Exempt Position 

Join ProHealth Care as the Coordinator RN – Remote Patient Monitoring and help transform how we support patients with chronic conditions. Based at Waukesha Memorial Hospital, this role combines hands-on nursing with innovative technology to improve patient outcomes and expand access to care.

What You Will Do:

As the RN Coordinator for Remote Patient Monitoring (RPM), you will play a key role in supporting patients with chronic health needs through proactive monitoring and care coordination. You'll guide daily workflow for the RPM team, while also stepping into direct patient care responsibilities when needed.

In this role, you will:

  • Monitor and triage patient readings, symptoms, and concerns through the RPM portal, escalating when necessary.

  • Educate patients on equipment use, health literacy, and self-management skills using motivational interviewing techniques.

  • Serve as a clinical resource for staff, supporting patient referrals, onboarding, and care coordination.

  • Partner with leadership to grow and enhance the RPM program, including marketing efforts and presenting to stakeholders.

  • Document accurately in the EMR and RPM software, ensuring compliance with protocols and billing requirements.

  • Lead daily team huddles and collaborate with inpatient teams to identify appropriate referrals.

  • Support weekend and holiday RN rotation as part of the department team.

This is NOT a fully remote position, hire will need to work onsite at Waukesha Memorial Hospital
