RVCC is looking to hire#a Program Supervisor for our Medical Case Management## Housing Program! River Valley Counseling Center, Inc#s mission is to improve the health of all people in our community. We do that through expert and compassionate care, education, community partnerships, fostering innovation, and by inspiring hope in all we touch.# We do that by being good stewards of our resources and providing efficient and cost-effective care for all. # Overview of Job Responsibilities: To supervise and lead all housing staff and medical case managers To assist in the recruitment, training, evaluation, and supervision of the on-the-job performance of assigned personnel To exhibit practical and technical knowledge successfully and to share that knowledge with other RVCC personnel seeking advice or assistance To assist in the monitoring, screening, and assignment of program referrals To attend meetings with funders and community partners as needed and identified by the program needs To assist in identifying, collecting, and reporting relevant statistical data that reflects the activities of the assigned programs and staff and to support compliance with administrative, regulatory, and if applicable, contractual requirements To develop and maintain the accuracy of client data within the agency#s electronic health record system, as well as the databases of the program funders To maintain accurate#and up-to-date data on all computer#systems as required by finders To conduct chart audits and ensure chart compliance as outlined by agency and/or funder requirements To keep the immediate supervisor informed of activities within the area of responsibility and to communicate needs on a timely and routine basis Job Requirements: Education Level -#Bachelor#s degree in Human Services, Social Work, Public Health or related health or human services field preferred. Years of Experience -#Minimum of 6 months experience working with individuals diagnosed with HIV/AIDS and knowledge of housing programs for homeless and at-risk populations.#Prior case management experience is required. Minimum 1-year supervision experience required. Experience working with electronic health records and databases is preferred.
