**Job Summary:**
We are seeking a compassionate and organized Care Manager to join our team. The Care Manager will be responsible for coordinating and managing comprehensive care plans for patients, ensuring their physical, emotional, and social needs are met. This role is crucial in facilitating communication between patients, families, healthcare providers, and community resources.

**Key Responsibilities:**

1. **Assessment and Planning:**
- Conduct initial and ongoing assessments of patients' health and social needs.
- Develop and implement individualized care plans in collaboration with healthcare providers, patients, and families.

2. **Care Coordination:**
- Act as the primary point of contact for patients and their families, providing information and support.
- Coordinate services and resources among various healthcare providers, community resources, and support services.
- Schedule and manage appointments, follow-ups, and transitions of care.

3. **Monitoring and Evaluation:**
- Regularly monitor patients' progress and response to care plans, adjusting as necessary to meet their evolving needs.
- Document outcomes and maintain accurate records of patients' care plans and services provided.

4. **Communication and Advocacy:**
- Facilitate communication between patients, families, and healthcare teams to ensure a clear understanding of care decisions and options.
- Advocate for patients' rights and needs to ensure they receive appropriate care and support.

5. **Education and Support:**
- Provide education and resources to patients and families about health conditions, treatment options, and available community resources.
- Offer emotional support and counseling to patients and families navigating complex health challenges.

6. **Collaboration:**
- Work collaboratively with interdisciplinary teams, including physicians, nurses, social workers, and other healthcare professionals.
- Participate in case reviews and team meetings to discuss patient progress and best practices.

**Qualifications:**

- Bachelor's degree in Nursing, Social Work, Public Health, or a related field (Master's degree preferred).
- Current licensure in Nursing or Social Work (if applicable).
- Prior experience in care management, case management, or a related healthcare setting.
- Strong knowledge of healthcare systems, community resources, and patient advocacy.
- Excellent interpersonal and communication skills, with the ability to build trusting relationships with patients and families.
- Organizational and time-management skills, with a strong attention to detail.
- Proficiency in using electronic health records (EHR) and other relevant software.

**Preferred Qualifications:**

- Certification in Case Management (CCM, ACM, or equivalent).
- Experience working with specific populations (e.g., geriatrics, chronic illness, mental health).
- Bilingual skills are a plus.
