Preventative Health Coordinator Details
Essential Job Duties:
- Utilizes reports to identify and schedule Annual Wellness Visits for assigned providers' patients.
- Prepares documentation for visits by reviewing patient charts, using systems like ClearView, WPS, and CHIRP.
- Conducts risk assessment screening via phone prior to Annual Wellness Visits.
- Demonstrates working knowledge of health records, computer systems, and data processing; manages paper charts, retrieves medical records, ensures accurate completion of paperwork, communicates effectively, and ensures HIPAA compliance.
Qualifications:
- Medical terminology required
- Minimum of one-year experience in a medical/healthcare setting required.
License/Certification:
- Medical Assistant certification preferred
- BLS Required
