VON - At the heart of care.   About the role:   As a Health Care Technician (HCT), you provide direct hands-on care to patients in the comfort of their homes under the guidance of a Directing Registered Nurse (DRN), whom you connect to virtually. This is an exciting opportunity for you to make a difference in the lives of patients of all ages, including those receiving end-of-life care, managing complex conditions, and pediatric patients. What you'll do: 

• Under the direction of the DRN:
• Observe and document the patient's electronic medical record
• Provide personal care support and assist with activities of daily living and medication administration
• Engage with the family/caregiver
• Identify additional resources required in the patient's home
• Communicate with the DRN through Smartphone Technology via a real-time electronic monitoring system
• Collaborate with the DRN to develop and modify the patient's care plan based on patient outcomes
• Perform other duties as assigned

What we are looking for:

• Personal Support Worker (PSW) certification
• Developmental Support Worker (DSW) certification will be considered
• One to two years' experience as a PSW
• Current CPR and First Aid certification
• Fundamentals of Palliative Care- Level I (or a commitment to complete within one year)
• Experience in Palliative or acute care (hospital) preferred
• AHPCE course completion is an asset
• Have a personal vehicle and possess both a current driver's license and proof of vehicle insurance
• A current and original copy of a satisfactory Criminal Records Check
• Must be able to wear Personal Protective Equipment (PPE)
