About the role: As a Health Care Technician (HCT), you provide direct hands-on care to patients in the comfort of their homes under the guidance of a Directing Registered Nurse (DRN), whom you connect to virtually. This is an exciting opportunity for you to make a difference in the lives of patients of all ages, including those receiving end-of-life care, managing complex conditions, and pediatric patients. - Under the direction of the DRN:
- Observe and document the patient's electronic medical record
- Provide personal care support and assist with activities of daily living and medication administration
- Engage with the family/caregiver
- Identify additional resources required in the patient's home
- Communicate with the DRN through Smartphone Technology via a real-time electronic monitoring system
- Collaborate with the DRN to develop and modify the patient's care plan based on patient outcomes
- Perform other duties as assigned
- Personal Support Worker (PSW) certification
- Developmental Support Worker (DSW) certification will be considered
- One to two years' experience as a PSW
- Current CPR and First Aid certification
- Fundamentals of Palliative Care- Level I (or a commitment to complete within one year)
- Experience in Palliative or acute care (hospital) preferred
- AHPCE course completion is an asset
- Have a personal vehicle and possess both a current driver's license and proof of vehicle insurance
- A current and original copy of a satisfactory Criminal Records Check
- Must be able to wear Personal Protective Equipment (PPE)
