This role supports people in developing life skills, running weekly classes, organizing community based experiences, connecting people to resources in the greater community, managing a caseload, completing treatment plans and orienting new people to the program. For a full description of benefit offerings, please visit: https://wellpower.org/workplace-of-choice/.</p> <p><strong>Responsibilities & Duties</strong>: </p> <p>· Outreach and maintain a caseload of individuals engaged in services</p> <p>· Assist people we serve with setting short and long-term learning and wellness goals</p> <p>· Complete data, reports and clinical documentation as required</p> <p>· Coordinate care between the rehabilitation department and the clinical team</p> <p>· Facilitate social activities and special events within the community and the program</p> <p>· Support individuals with educational opportunities and training within the program and community</p> <p>· Provide program education and orient new people to the program</p> <p>· Support peer navigation and provide peer support</p> <p>· Collaborate with individuals we serve to encourage engagement and support recovery</p> <p>· Provide services with a Trauma-Informed approach and crisis intervention and de-escalation when needed</p> <p>· Effectively collaborative with peers to oversee the day to day operations of the program</p> <p>· Be self-motivated and have good time-management skills</p> <p>· Model good communication, healthy working-relationships and appropriate boundaries</p> <p>· Support cultural diversity and encourage inclusiveness of people with different abilities</p> <p>· Be creative in bringing your strengths and interests to the team and program</p> <p>· Ability to transport people we serve to/from activities using agency van</p> <p>· Perform duties as assigned</p> <p><span style="text-decoration: underline;"><strong>Requirements & Qualifications: </strong></span></p> <p><strong>***Lived experience preferred</strong></p> <ul> <li>VC II: Bachelor degree in rehabilitation or related field.</li> <li>VC III: Masters degree in vocational rehabilitation or related field; with one year of experience in a vocational rehabilitation setting or experience with specific duties of this position is preferred.</li> </ul> <p><strong>Certifications: </strong>Certified Psychiatric Rehabilitation Practitioner or willingness to obtain certification within one year.</p> <p><strong>WORKING CONDITIONS: </strong> May require occasional evening or weekend hours.
