The care is provided under the direction of a physician, and the goal is to help patients regain their independence after an illness, injury or hospitalization. The goal for patients with chronic illnesses may be to keep them at home safely and as pain-free as possible. Services provided are Skilled Nursing Care, Home Health Aide Care, Social Worker Care, Physical Therapy Care, Occupational Therapy Care, and Speech Therapy Care.   Job Summary: The Home Health Care & Hospice Chaplain is supervises activities related to the spiritual, relational, and/or emotional needs of the Hospice patient/family.   Essential Duties & Responsibilities:
  • Provides assessment reflecting the spiritual and relational aspects of the patient and or caregivers situation through the spiritual assessment and visit.
  • Prepares a spiritual plan to address specific issues identified by the spiritual assessments.  Shares plan with appropriate staff members. 
  • Is an active member of the interdisciplinary team and participates in team conferences every two weeks.
  • Communicates impressions of ongoing visits and assessments with other staff involved in the case management as well as with outside clergy involved.  Offers assistance through participation in Hospice team conference.
  • Through work with the client/family assists them to constructively deal with the specific issues that are being addressed refers them to other resources as needed.
  • Completes required documentation on a timely basis.
  • Chaplain provides bereavement services including phone calls, visits and memorial services to family members upon the death of a patient.
  • Must be knowledgeable of and comply with Medicare Hospice conditions of Participation—Spiritual Caregiver.
  • Must be able to travel with in a 6 county radius 
  • Mileage Reimbursement
  EDUCATION and/or EXPERIENCEGraduate of accredited seminary or school of theology, or appropriate certification in hospital or pastoral ministry.  Master's degree in theology, divinity or pastoral studies.   One unit of Clinical Pastoral Education (CPE) and/ or experience in working with patients/families dealing with life threatening illness, and death.      Mission & Vision & Values:
